The Quality School Parent-Child Survey Instrument provides direct
customer feed back to individual teachers and the school as a whole.
Each teacher receives an individual confidential report. The
principal and support staffs receive school-wide reports. This is a
mail out survey. Completed surveys are returned via U.S. mail to the
